Vogelgrippe / Aviäre Influenza A (H5N1)

Die Vogelgrippe breitet sich aus. Das Deutsche Ärzteblatt hat bereits zweimal darüber berichtet [1].
Kommen wir zunächst einmal zu den positiven. Aspekten. Die Erkrankung ist bislang nur bei Tieren,  allerdings auch schon bei Säugetieren, aufgetreten. Ein Sprung des Virus hinüber zum Menschen ist bislang nicht erfolgt. Wir hatten allerdings im Zeitraum 2006 bis 2010 Erkrankungen der Vogelgrippe beim Menschen. Der letzte Eintrag vom RKI ist vom 12.08.2011 [2]. Dort wurden u.a. das Monitoring System, das 2005 eingerichtet worden ist, vorgestellt sowie ein wissenschaftlicher Artikel und weitere Informationen eingestellt. Ich hatte mich früher mit der Vogelgrippe bereits mehrfach auseinandergesetzt, aber es handelte sich damals um andere Varianten [3].

Die Vogelgrippe oder aviäre Influenza A (H5N1) wurde im Jahr 1996 entdeckt und zwar im südlichen China und Hongkong. Dieses Virus tötet einen großen Anteil von infiziertem Geflügel. Manchmal kommt es zur Übertragung von Vögeln auf den Menschen. Es kann eine sehr schwere Erkrankung beim Menschen hervorrufen. Bislang hat es wenig Auswirkung in Europa gehabt, jedoch gelangte 2021 das Virus stärker nach Europa und hatte zu einer Epidemie 2021/2022 geführt, bei der mehr als 50 Mill. Vögel infiziert worden waren [4].

Nun, die Aufregung hält sich noch in Grenzen, aber es sind eben in Nerzfarmen Übertragungen aufgetreten, nicht nur von Vögeln zu Nerzen, sondern auch zwischen Nerzen. Die Anpassung des Virus an den Nerz bedeutet nicht sofort eine Gefahr für den Menschen. Wir hatten bereits früher die Schweinegrippe und diese, beziehungsweise die Übertragung der Vogelgrippe auf Schweine, ist insofern viel interessanter, da wir dem Schwein genetisch sehr viel näher stehen, als uns das manchmal lieb ist. Und daher wäre eine Übertragung von Schwein zu Mensch und dann von Mensch zu Mensch sehr viel eher denkbar.  

Forscher äußern sich besorgt über die höhere Viruslast, globale Ausbreitung und die schnellere Fähigkeit im Vergleich zum Coronavirus zu mutieren, so daß dadurch auch eine Gefahr für den Menschen bestehe. Professor Christian Drosten relativiert das, da es ist zu keiner besorgniserregenden Häufung der Übertragung auf Menschen, geschweige denn zur Übertragung von Mensch zu Mensch gekommen ist. Begrenzte Infektionen zwischen Menschen sind allerdings  in der Vergangenheit beschrieben worden. Diese sind aber viel geringer, als daß sie den Anfang einer Pandemie darstellten. Auch wenn eine Anpassung des Virus an den Menschen nicht vorher bestimmbar ist, engagiert sich Drosten für eine technisch machbare und vom Aufwand vertretbare Vorbereitung zur Impfstoffentwicklung.

Zusammenfassend kann man die Bedrohung aktuell als gering erachten. Aber am Ball bleiben müssen wir doch, damit sich nicht eine weitere Pandemie entwickelt.

Biologische Impfung als sanfte Alternative?


Eine meiner Patientinnen fragte mich, ob sie die Biologische Impfung bzw. Paracelsus Grippe Impfung, die ihr von der Heilpraktikerin angeboten worden ist, durchführen soll. Heilpraktiker dürfen keine Impfungen durchführen! Entweder wird hier wider geltendes Recht gehandelt (es ist eine Impfung, die von der Heilpraktikerin durchgeführt wird) oder es ist eine Mogelpackung (keine Impfung).

Ich recherchierte, dass die Biologische Impfung bzw. Paracelsus Grippe Impfung eine Kombination homöopathischer und naturheilkundlicher Präparate ist, welche als Injektion und Eigenblutbehandlung verabreicht werden.

Ich lese: „Die Ärzte der Paracelsus-Klinik raten von der alljährlich überall propagierten Grippe-Impfung ab, da sie Lebend-Virus-Partikel enthält und nur gezielt gegen eben einen Virus schützt, von welchem man annimmt, dass er die Grippewelle des entsprechenden Jahres verursache.“ Die Influenza-Impfung schütz nur gegen Influenza und zwar verschiedene Stämme. Das ist sinnvoll, denn die Influenza hat eine hohe Rate an Todesfällen. Lebende Virus-Partikel werden in der Regel nicht geimpft.

„Zudem schützt die konventionelle Grippeimpfung gegen alle andern Viren, die ohnehin die meisten Erkältungen bewirken, in keiner Weise.“ Und aus der Zapfsäule der Tankstelle fließt keine Milch. Die Grippe (Influenza) hat eine hohe Rate an Sterblichkeit, der grippale Infekt (banale Erkältung) nicht. Die Spanische Grippe forderte 25-50 Millionen Menschenleben; prominentes Opfer der Großvater von Donald Trump.

Weiter lese ich: „Die «Paracelsus-Grippe-Prophylaxe» ist keine «Impfung», sondern eine Immunstimulation und Nosodentherapie …“. Warum dann nennt man es Impfung? Es handelt z.B. um homöopathische Präparate und „Ancopir (ein Vitamin-B-Komplex-Präparat, mit B12 und Langzeitwirkung)“.

Gibt es denn eine Studie, die belegen würde, dass die Hypothese (es findet eine Immunstimulation statt) oder die Hypothese (mit an Sicherheit grenzender Wahrscheinlichkeit kommt es nicht zu einer Grippe oder zu einem grippalen Infekt) zutrifft? Ich habe keine gefunden, und mit an Sicherheit grenzender Wahrscheinlichkeit gibt es so eine Studie auch nicht.

Die biologische Impfung ist keine Impfung sondern eine Mogelpackung.
Eine Wirksamkeit ist nicht nachgewiesen.
Die Gefahr kann aus der Nicht-Impfung gegen Influenza entstehen.
Ich riet ab.

La vaccination biologique comme alternative douce?

Une de mes patientes m'a demandé si elle devrait faire le vaccin Bioculture ou Paracelsus Influenza proposé par le médecin alternatif. Les naturopathes ne sont pas autorisés à vacciner! Soit cela est fait ici contrairement à la loi en vigueur (il s’agit d’un vaccin qui est réalisé par le praticien alternatif) ou il s’agit d’un simulacre (pas de vaccin).

J'ai étudié le vaccin Bioculture ou Paracelsus Influenza comme une combinaison de préparations homéopathiques et naturopathiques administrées par injection et par traitement du sang autologue.

Je lis: "Les médecins de la clinique Paracelsus déconseillent le vaccin antigrippal à propagation annuelle, car il contient des particules virales vivantes et ne protège spécifiquement que contre un virus, qui serait la grippe de l’année correspondante. "La vaccination antigrippale ne protège que contre la grippe et même contre différentes souches. Cela a du sens, car la grippe a un taux de mortalité élevé. Les particules virales vivantes ne sont généralement pas vaccinées.
"En outre, le vaccin antigrippal classique protège contre tous les autres virus, qui causent le plus froid de toute façon, de quelque manière que ce soit." Et de la pompe à essence de la station-service ne coule pas de lait. La grippe (grippe) a un taux de mortalité élevé, pas l’infection grippale (rhume banal). La grippe espagnole a fait entre 25 et 50 millions de morts. victime de premier plan du grand-père de Donald Trump.

Ensuite, je lisais: "La" prophylaxie contre la grippe Paracelsus "n’est pas un" vaccin ", mais une stimulation immunitaire et une thérapie nosode ...". Pourquoi appelez-vous cela la vaccination? C'est par exemple préparations homéopathiques et "Ancopir (une préparation de complexe de vitamine B, avec B12 et effets à long terme)".

Y a-t-il une étude qui prouverait que l'hypothèse (il y a une stimulation immunitaire) ou l'hypothèse (cela ne vient presque certainement pas d'une grippe ou d'une infection grippale)? Je n'en ai trouvé aucune et il est presque certain qu'aucune étude de ce type n'a été réalisée.

Le vaccin biologique n'est pas un vaccin mais un simulacre.
Une efficacité n'a pas été prouvée.
Le danger peut provenir de la non-vaccination contre la grippe.
J'ai déconseillé.

2017 and the Large H7N9 Avian Flu Outbreak




I had been interested in the H7N9 avian flu and started to write on this topic in April 2013 (1). But since November 2016 there have been new cases in Southern China. As of the 3rd of January 2017 the WHO counted „a total of 809 laboratory-confirmed human infections with avian influenza A(H7N9) since early 2013”. Wikipedia has counted 1,223, also since 2013 but as of 2nd of March, which makes it more than 400 cases during the past two months. Better work on the number of infected people you will find here: “人感染甲型H7N9禽流感病毒中国“(4). “Between January 19 and February 14, 2017, the Chinese mainland communicated to the WHO 304 cases of newly diagnosed laboratory infections through the National Sanitary Regulations National Focal Point.”

The virus was again first seen in poultry in live bird markets in Southern China, but the Chinese authorities closed these markets, which slowed down new infections. Most of the infected people had direct contact with infected birds, mostly chickens. The virus will unlikely spread from human to human.

Bird might cover great distances, which makes the influenza H7N9 virus dangerous. As the numbers of newly infected people in China is declining, let’s hope, the outbreak stops soon.

Vogelgrippe-Virus H5N8





Das Vogelgrippe-Virus H5N8 ist bislang bei Wildvögeln Schleswig-Holstein und am Bodensee in Deutschland nachgewiesen worden. Außerdem wurde das Virus in einem Geflügel-Großbetrieb in Grumby nachgewiesen, so dass mit der sogenannten Keulung (Tötung) von 30.000 Tieren begonnen wurde. Das Virus breitet sich über Zugvögel aus und zwar von Indien (Anfang Oktober) über Ungarn und Polen (Ende Oktober); wobei mir unklar ist welche Route die Vögel da ziehen. Ungewöhnlich ist aber auch, dass viele Tiere daran sterben.


Wie sieht es mit der Gefahr für Menschen aus? Menschen sind bislang nicht betroffen und die Gefahr wird vom Robert Koch Institut (RKI) als gering eingestuft. Andererseits teilt das RKI mit: „Ärzte sollten sich der Möglichkeit von sporadischen Infektionen mit nicht-humanen Influenzaviren bewusst sein. Insbesondere bei Häufungen von Atemwegserkrankungen und bei Auftreten außerhalb der regional üblichen Influenzasaison sollten Verdachtsfälle explizit zu ihrer beruflichen Exposition gegenüber Tieren befragt werden.“


Ich habe mir sicherheitshalber noch einige chinesische Seiten angesehen. 目前並沒有足夠科學證據顯示,人用H5N1流感疫苗對於H5N2H5N3H5N8禽流感病毒具交叉保護效果Dieser interessante Hinweis beschreibt einen gewissen Schutz der Impfung gegen H5N1 bei H5N2, H5N3 und H5N8. Z.B. Influvac® Saison 2016/2017 impft gegen H1N1 und H3N2 [genauer: A/California/7/2009 (H1N1) pdm09 – ähnlicher Stamm (A/California/7/2009, X-181), A/Hong Kong/4801/2014 (H3N2) – ähnlicher Stamm (A/Hong Kong/4801/2014, X-263B) und B/Brisbane/60/2008 – ähnlicher Stamm (B/Brisbane/60/2008, Wildtyp)].


Nach dem RKI wurden bislang Erkrankungen von Menschen mit dem hochpathogenen Vogel- Influenzavirus des Subtyps A (H5N8) bisher nicht beobachtet, aber gänzlich ausgeschlossen werden kann dies nicht.

Schweinegrippe im Anmarsch?



Über Medscape Deutschland Infektiologie wurde ich über eine mögliche neue Schweinegrippewelle informiert. In den U.S.A. ist nämlich eine Grippewelle ins Rollen gekommen. Als Erregertyp des Influenza A Virus wird H1N1 pdmo9 genannt. Dies ist aber der Erreger, der 2009 die Pandemie ausgelöst hatte, die bei uns unter Schweinegrippe geführt wird.
DIE WELT berichtete über den „Ausbruch einer neuen Schweinegrippe-Epidemie mit mehr als 40 Infektionsfällen im Norden Spaniens“. Ein Mensch ist daran gestorben.
Im April / Mai dieses Jahres berichtete ich über den Ausbruch einer Vogelgrippe-Epidemie in China (New strain of bird flu - H7N9). Bis zum 10.05.2013 waren 130 Menschen erkrankt, von denen 32 verstorben sind.
Eine Grippewelle im Februar 2013 war eher eine Welle in der Presse.

Wird es nun ernst? Werden wir wieder wochenlang über Gefahren informiert, die sich dann in Luft auflösen? Wird wieder Hysterie herrschen? Ich hoffe nicht. Trotzdem gilt es, die Lage in den Vereinigten Staaten zu verfolgen. Es sind wieder jüngere Menschen als bei den üblichen Grippewellen betroffen. Die von der „World Health Organisation (WHO) für die nördliche Hemisphäre empfohlenen tri- und quadrivalenten Impfstoffen dieser Saison enthalten“ auch eine Komponente, die gegen den Stamm H1N1 pdm09 A/California/7/2009 immunisiert. Für Deutschland meldet das Robert-Koch-Institut nur die sogenannte Hintergrundaktivität, jedoch keine Häufung. Die Centers for Disease Control and Prevention (CDC) in den U.S.A. melden für die zweite Januarwoche „weiträumige Influenza-Aktivitäten in 35 US-Staaten und damit in 10 Staaten mehr als eine Woche zuvor“. Es wurden den CDC 2.486 Influenza-Erkrankte gemeldet.

Insgesamt kein Grund zur Sorge, aber vielleicht könnten dies zum Anlass nehmen, sich gegen Influenza impfen zu lassen. Risikopersonen sind die über 60jährigen, Schwangere, chronisch Erkrankte (z.B. rheumatische Erkrankungen) und Personen, die beruflich viel mit Menschen zu tun haben (siehe Link).

H7N7 – a new strain of avian flu virus found in China (H7N7病毒)



According to the Epoch Times (link see below) there’s a new strain of pneumonia virus (H7N7), which might be more dangerous than the H7N9 strain, which caused 135 infections and 40 deaths earlier this year (研究:中国市场发现H7N7病毒. 更易感染给人).
The new strain can kill birds and ferrets alike, which means that it can also spread to humans. The new strain has been found while checking  poultry for H7N9. Prof. Guan Yi of Hongkong University and his team found the new strain H7N7 in Wenzhou City of Zhejiang Province.  
Sohu Health quotes Bitish media, that “new studies confirm H7N9 avian flu virus originated from chickens” (新研究确认H7N9禽流感病毒源自家鸡).
There’s also an article in the Guardian (luckily this laptop hadn’t been smashed to pieces): “New H7N7 bird flu strain discovered that could pose threat to humans”.
Right now there hasn’t been a report on humans being infected. Let’s hope it stays this way.

New strain of bird flu - H7N9


Update 06.12.2015:
There isn't much new to tell. CDC updated about two weeks ago: http://wwwnc.cdc.gov/travel/notices/watch/avian-flu-h7n9-china. "According to the World Health Organization, 667 cases and 229 deaths from H7N9 flu have been reported globally from March 2013 through October 14, 2015, most from the mainland of China."

A new strain of bird flu has caused 5 deaths of humans and infection of 12 people so far. Now all poultry is slaughtered at a Shanghai market. This new strain of bird flu H7N9 will be harder to detect than H5N1 as birds might be infected without developing the disease. As pigeons had also been infected, the disease might spread beyond raised poultry.
There are different serotypes of influenza A H1N1 caused the Spanish flu in 1918 and swine flu in 2009, H2N2 caused the Asian flu in 1957, H3N2 the Hongkong flu in 1968, etc. H5N1 caused the bird flu in 2004/2005. Flu might spread easily and beyond species, but we are already past the peak season for influenza … on the Northern hemisphere. If this bird flu starts causing problems there will be at least enough time to develop vaccinations. The swine flu in 2009 caused more problems by hysteria than by the disease itself, at least here in Germany. Our hospital had to make precautions, but we didn’t ever have one patient admitted because of the swine flu.
Let’s look at the new strain of bird flu with interest but without hysteria.

Health authorities are worried now about a 4 year old boy, who is a healthy (asymptomatic) carrier of H7N9. See: http://edition.cnn.com/2013/04/16/world/asia/china-birdflu/index.html. Healthy carriers are problematic, because you can't identify them easily, which is essential for disease control. China is quite rigorous in applying quarantine, which would be essential for stopping the disease to spread. With "71 confirmed cases of H7N9 and 14 confirmed deaths" it is a serious disease. But until now, even if there is an asymptomatic carrier, human to human infection of H7N9 hasn't been noticed up to now.

Phoenix net (凤凰网) informs in an article about Shanghai. There have been 32 infected persons in the vicinity of Sghanghai, 11 people died, 16 are treated under quarantine, and 5 persons have been dismissed from hospital after treatment. One female patient aged 80 fell ill on the 9th of April, had been admitted to Shuguang-Hospital (曙光医院) on the 16th of April, and diagnosis of H7N9 influenza has been confirmed on the 17th of April; seven close contacts of this patient are under medical observation.

The discussion has already been opened wheter H7N9 can also be transmitted from human to human. Still this hasn't been confirmed and so it seems that humans only get infected by infected poultry. See also: http://www.cbc.ca/news/health/story/2013/04/19/bird-flu-h7n9.html.

The CDC issued an "Interim Guidance on the Use of Antiviral Agents for Treatment of Human Infections with Avian Influenza A (H7N9)", in which an "antiviral treatment with a neuraminidase inhibitor as early as possible" is recommended. The public health agency Canada's Federal Government recommends people, who travel to China to avoid contact with live poultry or other birds in markets. Returning back to Canada and suffering from fever, and cold symptoms, one should left best telephone the family doctor instead of going to his practice or a hospital.

I think that't a good precaution in avoiding a spread of the disease.

Translation: Male patient, 60 years of age, now lives in Wuxi City Binhu district. The 5th expert group in Jiangsu Province diagnosed him as suspected case of human infection of H7N9 avian influenza. On the 26th H7N9 avian influenza viruses were isolated in specimens from the patient, and provincial experts team diagnosed the case as confirmed case of human infection H7N9 avian influenza . Currently under treatment in a hospital in Wuxi, in critical condition.

Whereas China has been accused in the past to be too silent about SARS, nowadays we flooded with very detailed infomation.

截至4月26日11时,内地已确诊H7N9禽流感病例112例,23人死亡,15人痊愈。Until the 26th of April there were 112 confirmed cases, with 23 patients having died, 15 patients have recovered.

It shows, that there is more transparency in China than for instance during the SARS endemic. The article commented on the fact, that infected bird must not necessarily show signs of the disease. The article cautions about the transparency issue, but close that "there is no doubt that technology makes it increasingly difficult to hide sensitive information".
I see it a bit more favourable on the Chinese side - but maybe my sinophily leaves me a bit prejudiced.

"Since H7N9 is not spreading easily from person to person at this time, CDC does not recommend that people delay or cancel trips to China. The World Health Organization also is watching this situation closely and does not recommend any travel restrictions."
I like it, if people in charge don't act hysterically! The recommendations will surely change if this strain of avian flu combines with a human flu strain and learn how to spread from human to human. As this might not happen at all, we don't have to worry today.
In Chinese news there are only a few new infections, now also in Fujian province.

03.05.2013:
On http://health.gmw.cn/dzb/2013-05/03/content_1302544.htm there is an article ( 两大监测网及时捕捉H7N9) comparing SARS and the recent human infection h7n9 avian influenza outbreak. The article is based on an expert meeting in Beijing this week and gives an account of the history of this outbreak. It tells on how and how many tests were spread in the beginning (early April). A monitoring management information system was set up and updated.
Feng Zijian, Director of China's CDC (Centers for disease control and prevention) was quoted that China's public health emergency response system for outbreak detection reacted to the H7N9 outbreak in a timely manner, concerning confirmation of the disease, and taking control measures, including emergency mechanisms.
Even taking into accound that it is an official publication, China has done much better than in the past. And I didn't detect the hystery we saw with the swine flu here in Germany/Europe in 2009/2010.

Interesting is one patient in Hunan province, Yueyang, who had already died on April 1st:
"50-year-old patient, gender unknown. Hospitalised on March 26. Diagnosed as having been infected with the new H1N1 strain of flu virus, not the H7N9 strain. Died on April 1." His fate shows the dangers of influenza. People may die from this disease, also from other strains.

"Nicoll added that he thought the Chinese were doing an "impressive job" handling, reporting, investigating and seeking to contain the outbreak."

国外专家如何看H7N9 - How do the Foreign Expert look at H7N9?
I'll refer to a few highlights:
H7N9, H5N1, H1N1 are obvious different. H7N9 and H5N1 spread mainly in animals, and occasionally infects humans. H5N1 is highly pathogenic to poultry, but H7N9 avian flu symptoms are relatively mild in poultry and therefore less likely to be found.

The SARS outbreak and recent H7N9 in China both have their own specific reasons.
The virus might survive normal cooking temperatures, but it's safe as long as all parts of the food reach 70° C. (However, the expert didn't state for how long - as as most of the readers won't have this problem, it's academical.)

Experts at Fudan University in Shanghai (复旦大学上海医学院) found H7N9 virus "nucleic acid" in individual patients after 19 days of treatmen and published the data in The Lancet (the article of The Guardian is based on this data). ScienceNet China reports: OIE spoke highly of China H7N9 avian influenza prevention and control (世界动物卫生组织高度评价中国H7N9禽流感防控). At 81st session of the International Conference the Chinese Government has been congratulated for having taken firm and decisive measures to timely and effectively reduce H7N9 avian flu risk in just two months. Shangdou.net (尚都网) published an article: 中国H7N9疫区应急响应全终止 (China terminated H7N9 area emergency response). China terminated full emergency response in H7N9 infected areas. This does not mean that the virus disappeared. The risk for infection still exists in the South are of China. Daily monitoring of H7N9 in poultry will continue. Liao Ming (廖明), the Vice President of the South China Agriculture University, stresses the need to strengthen the monitoring of birds and outbreaks of human infection with avian influenza.
