Methotrexate (MTX) and Caffeine

 



Recently I had written a blog post on Methotrexate (MTX) and Caffeine in German [1]. This is an offspring and not merely a translation of this article.

Klaus Krüger, a German Professor of Rheumatology based in Munich, told in an interview for BDI aktuell [a publication of the BDI, which stands for Berufsverband Deutscher Internisten - Professional Association of German Internists], that even though MTX is generally well tolerated, nausea is a common side effect. He mentioned coffee being able to alleviate this side effect.

As I've known Dr. Krüger for at least 15 years, I thought to look for the study not mentioned in BDI aktuell. He monitores, condenses and lectures on studies and won't give this kind of information without having read a study. And I've found a likely study by Dr. Anand Narayan Malaviya [2].

In his study, A.N. Malaviya examined the effect of caffeine on the symptoms of methotrexate intolerance in patients with rheumatoid arthritis. Caffeine (coffee / dark chocolate) relieved the symptoms of MTX intolerance in the majority of patients.

Let's look at some details of this study, which looked at 855 patients treated with methotrexate. 313 patiens did not have any MTX intolerance, leaving 542 patients with some degree of MTX intolerance. In 422 patients MTX intolerance did not require any intervention. 120 (14 % of the initial 855) patients had 'moderate' or 'severe' MTX intolerance. “Among these, 55 % had complete relief of symptoms and were able to continue taking the advised dose of MTX; 13.3 % had partial improvement and continued taking MTX but only with antiemetics; 7.5 % were minimally better but were somehow managing; 10 % were complete caffeine failure without any relief; 14.2 % did not like caffeine (coffee or dark chocolate) and did not want to take it.” Tea, not coffee is mostly drunk in the northern part of India.

I myself like coffee and dark chocolate, but I would have no hesitation in recommending tea drinkers to give it a try; however, precisely that was not tested. Dark chocolate and coffee contain slightly more caffeine when compared to tea. However, it is more important that the polyphenols in tea (e.g. epigallocatechin gallate) bind the caffeine and only release it in the intestine, which renders the kinetics different from those of coffee, for example. It may well be the case that the faster absorption of caffeine from coffee is more effective at preventing methotrexate nausea.

I've always been concerned about strategies to keep patients on MTX as it is very effective in itself or as a co-medication [3]. Give coffee a chance!

Methotrexat (MTX) und Kaffee

 

In der neuesten Ausgabe von BDI aktuell stand ein Artikel, in der Prof. K. Krüger, Rheumatologe in München, zitiert wurde [1]. Im Artikel steht: „Bei Übelkeit, einer der häufigsten Nebenwirkungen des sehr gut verträglichen MTX, ist wegen einer wohl lindernden Koffein-Wikung Kaffee einen Versuch wert.“

Ich dachte an zwei Dinge: 1. Probieren auf jeden Fall, 2. wie sicher ist der Hinweis vom Kollegen Krüger? Ich hatte vorher noch nichts davon gehört. Früher hatte ich hier auf dem Blog schon über eine Verbesserung der Verträglichkeit von MTX geschrieben [2]; „[j]eder Tipp, um Nebenwirkungen zu lindern und den Menschen auf dem Medikament halten zu können, ist ein wahrer Segen“.

Und nun die frohe Botschaft: es gibt eine Studie [3]. In seiner Studie untersuchte A. Malaviya die Wirkung von Koffein auf die Symptome einer Methotrexat-Intoleranz bei Patienten mit rheumatoider Arthritis. Koffein (Kaffee/dunkle Schokolade) linderte die Symptome der MTX-Intoleranz bei 55 % und teilweise bei 13 % der Patienten. 14,2 % mochten weder Kaffee noch dunkle Schokolade, da im nördlichen Teil Indiens hauptsächlich Tee getrunken wird und nicht Kaffee. Vielen Dank Anand Narayan Malaviya und Klaus Krüger!

Ich hätte keine Bedenken, Teetrinkern zu empfehlen, es einmal zu versuchen; allerdings wurde gerade das nicht getestet. Dunkle Schokolade und Kaffee haben einen leicht höheren Gehalt an Koffein, wenn man die Produkte mit Tee vergleicht. Wichtiger ist aber, daß die Polyphenole im Tee (z.B. das Epigallocatechingallat) das Koffein binden und es erst im Darm freilassen, d.h. die Kinetik ist anders als etwa beim Kaffee. Es kann durchaus sein, daß die schnellere Aufnahme von Koffein aus Kaffee die Übelkeit seitens Methotrexat besser verhindert.

PS. BDI – ich war einmal beim Bundesverband der Deutschen Industrie e.V. in Köln in den 1970iger Jahren, um ein Buch von Arno Schmidt einzusehen. Ich kam mir vor wie der Reporter in Citizen Kane. Aber dieser BDI ist nicht gemeint sondern der Berufsverband Deutscher Internisten.

CR6086 at the 2020 EULAR Online Meeting


When I've checked for novel drug candidates, I've stumpled on CR6086. Somehow the number reminded me of Intel's 8086-series and others might associate Nokia's 6086-series. But CR6086 is an EP4 [a prostanoid receptor] receptor antagonist, which “may improve the response of tumours to immuno-oncology therapies, e.g. immune checkpoint inhibitors such as anti-PD-1 and anti-CTLA-4 drugs” [1]. CR6086 wouldn't be the first drug that proves useful not only in oncology but also in rheumatology.

There had been a study at the 2016 ACR/ARHP Annual Meeting [2], in which the authors concluded: "CR6086 was safe up to the maximum tested dose of 300 mg."

Adis Insight [3] has data up to March 28th 2020 (“No recent reports of development identified for phase-I development in Rheumatoid-arthritis(In volunteers) in Italy (PO)”), and mentions two dates for study NCT03163966 in the Czech Republic (3rd November 2017) and the preliminary data presented at the 2018 ACR/ARHP Annual Meeting as CREATIVE trial data [4]. Now there is data on study NCT03163966.

K. Pavelka and colleagues presented the following study [5] at the 2020 EULAR Online Meetin: „AB0360 EFFICACY AND SAFETY OF THE PROSTAGLANDIN EP4 RECEPTOR ANTAGONIST CR6086 ADDED TO METHOTREXATE IN DMARD-NAIVE EARLY RA PATIENTS: A PHASE 2 RANDOMIZED CONTROLLED TRIAL“. The authors concluded: „There was no benefit demonstrated for CR6086 added to MTX in the study cohort as a whole. However, in a post-hoc analysis, enhanced responses were observed with CR6086 90mg bid added to MTX in patients >6 months disease duration. This generated the hypothesis that addition of CR6086 90mg bid may benefit in RA patients initiating MTX after the window of opportunity, to be tested in further studies.“ There aren't any irregularities in methods and results. Even without post-hoc analysis there's a difference for MTX with or without CR6086. But 'll show you, why I habor doubts.

I've looked at a study by G. Caselli and colleagues [6]: „Pharmacological Characterisation of CR6086, a Potent Prostaglandin E2 Receptor 4 Antagonist, as a New Potential Disease-Modifying Anti-Rheumatic Drug“. In results the authors tell us: „ In models of human immune cells in culture, CR6086 reduced key cytokine players of RA (IL-6 and VEGF expression in macrophages, IL-23 release from dendritic cells, IL-17 release from Th17 cells). In the CIA model of RA in rats and mice, CR6086 significantly improved all features of arthritis: severity, histology, inflammation and pain. In rats, CR6086 was better than the selective cyclooxygenase-2 inhibitor rofecoxib and at least as effective as the Janus kinase inhibitor tofacitinib.“ But there's no hint to what happens to MTX if combined with CR6086.
J.M. Kremer and R.A. Hamilton published in 1995 [7]: „The effects of nonsteroidal antiinflammatory drugs on methotrexate (MTX) pharmacokinetics: impairment of renal clearance of MTX at weekly maintenance doses but not at 7.5 mg“. So NSAIDs alter renal clearance of MTX at normal weekly maintenance doses. In a more recent study (2011) Yuichi Uwai and colleagues [8] showed in their meta-analysis „that NSAIDs increase blood levels of methotrexate by influencing renal excretion of the antifolate“.
Now, CR6086 isn't an NSAID, but it may (ot may not) effect the renal clearance of MTX.

K. Pavelka and colleagues stated the „hypothesis that addition of CR6086 90mg bid may benefit in RA patients initiating MTX after the window of opportunity“. I have my doubts, but I agree that this has to be tested in further studies. Hopefully, K. Pavelka and colleagues are right.

Überdosierung Folsäure


Überdosierung Folsäure? Das Vitamin ist eher dafür bekannt, dass ein Defizit besteht. In der Rheumatologie hat es eine besondere Bedeutung, da wir es bei der Therapie mit Methotrexat (MTX) substituieren, damit es erst gar nicht zu Nebenwirkungen kommt. Während der durchschnittliche Erwachsene 300-400 µg täglich zu sich nehmen sollte, werden beim Rheumapatienten 5-10 mg einmal wöchentlich gegeben. Deshalb ist die Frage nach einer Überdosierung gerechtfertigt.

Folsäure gehört zu den Vitaminen aus der Gruppe der B-Vitamine und wird manchmal auch als Vitamin B9 bezeichnet (historisch, heute sollte man bei Folsäure bleiben). Folsäure ist z.B. in Spinat, Grünkohl, Rosenkohl, Weizenkeimen, Bohnen, Kichererbsen, Soja, Hefe, Vollkornbrot, Eigelb und weiteren enthalten. Manches Salz ist mit Folsäure, Fluorid und Jod angereichert. Aber zu viel kann auch schaden. Ein Folsäureüberschuss kann einen Vitamin-B12-Mangel verdecken; Schäden an den Nervenscheiden können u.U. schon irreversibel sein.

Eine Überdosierung von Folsäure kann das Herzinfarktrisiko erhöhen. Die Einnahme von hohen Dosen an Folsäure wurde mit Lungen- oder Prostatakrebs in Verbindung gebracht.

Mögliche Symptome einer Überdosierung können sein: nervöse Zustände, Depression, Hautrötung, Hautausschläge, Magen-Darm-Problem.

Eine Überdosierung an Folsäure ist möglich – kommt allerdings nur selten vor. Sie sollten ggf. Rücksprache mit Ihrem behandelnden Arzt nehmen und nicht die Therapie ohne diesen Rat verändern.

WIN-Session SLE beim Kongress der DGRh 2018 in Mannheim



Im Laufe der letzten 25 Jahre wurde die Bedeutung der Antimalariamittel für die Langzeitprognose entdeckt. Mycophenolat Mofetil (MMF) ist eine Alternative zu Cyclophosphamid. Rituximab ist besonders bei Antikörpervermittelten Symptomen hilfreich. Eine konsequentere Kortikosteroid Reduktion muss angestrebt werden. Calcineurininhibitoren sind bei Lupusnephritis wirksam. Belimumab wurde zugelassen.
Konsequenter Sonnenschutz (durch Bekleidung und Sonnenschutzcremes mit LSF über 50). Vitamin D3 mit 2000-5000 IE täglich zu fettreichem Essen. Kalzium diätetisch und durch Substitution  sollte mindestens 1,2 g pro Tag betragen - Vorsicht bei Niereninsuffizienz sowie Nierensteinen. Gute Paitentenaufklärung verbessert die Compliance. Kariovaskuläre Risiken vermidern. Patienten sollten selbstständig Urin mit Teststäbchen untersuchen. Impfungen durchführen lassen.
Off-label: MTX, Leflunomid, Cyclosporin A, Tacrolimus, Rituximab; MMF nur bei Lupusnephritis zugelassen.
Wir benötigen eine Treat-to-Target Strategie mit geeigneten und validierten Aktivitätsscores.
Anifrolumab (IFN-Typ1-Rezeptorblockade) hat in einer Phase 3 Studie den primären Endpunkt verfehlt. Baricitinib, IL-2, Ustekinumab werden zur Zeit untersucht, da bereits positive Daten vorliegen.

Das Makrophagen Aktivierungs Syndrom kann aussehen wie der Schub einers SLE. Red flags sind: hohes Fieber, Erhöung von CRP, Procalcitonin, Ferritin, GOT, LHH, Neutopenie, Myokarditis, Krampfanfälle.
Initial sind hochdosiert Glukokortikoide notwendig, gefolgt von Cyclophosphamid oder Etoposid.
Risikofaktoren vür die Aufnahme auf die Intensivstation sind Thrombozytopenie und hohes CRP.

Es wurde die Frage "Brauchen wir Belimumab?" gestellt und mit: Ja, aber beantwortet.

Methotrexat auf dem Kongress der DGRh 2018 in Mannheim




Methotrexat (MTX) ist für mich und bestimmt alle Rheumatologen, aber auch Gastroenterologen, Dermatologen und Onkologen eine faszinierende Substanz. Wenn Sie aber in die Internetforen gehen, so werden Sie viel über Nebenwirkungen hören. Ja, pharmakologisch wirksamen Substanzen gibt es nur mit der Möglichkeit der Nebenwirkung oder der unerwünschten Wirkung einher. Nur pharmakologisch unwirksame Substanzen haben auch keine Nebenwirkungen, wenn man einmal die Nebenwirkung einer unbehandelten Erkrankung außen vor läßt. Methotrexat (MTX) hat ein besonders gutes Verhältnis von Wirkung und Nebenwirkung; außerdem ist es gut steuerbar. Es ist das Medikament in der Rheumatologie, bei dem wir auf die größte Menge an Erfahrung blicken können.

Methotrexat ist ein Folsäure-Analogon. Ein anderer Name ist Amethopterin. Klingt eher wie eine ägyptische Gottheit. Über Namen kann man bekanntlich streiten. Aktuell kommen Präparate Namen wie Taltz, Olumiant oder Kevzara auf den Markt. Bei Taltz denkt man eher an Kaltz und seine Bananenflanke, bei Kevzara denke ich eher an Keflavik, den Flughafen von Reykjavik, bei Olumiant verknotet sich meine Zunge. Ich habe einmal über eine oral einzunehmende Lösung mit dem Namen Jylamvo® geschrieben. Ich über legte damals, ob man ein Medikament nicht gleich Grzpflzly® nennen sollte.
Bei Methotrexat ist die Abkürzung MTX bei Ärzten wie Patienten geläufiger. Unten als letzter Link habe ich auf einen Artiekel hier auf dem Blog verwiesen, der sich (auf Englisch) mit der Wirkungsweise beschäftigt.

Auf dem Kongress der DGRh 2018 in Mannheim gabe es ein Satellitensymposium unter dem Titel " Methotrexat — zentraler Baustein der modernen Rheumatherapie". Der Vorsitz hatte Prof. Dr. med Markus Gaubitz.

M. Gaubitz bestritt auch den ersten Vortrag, der auch der wichtigste war in Hinblick auf Methotrexat. MTX wir in der Rheumatologie seine 30 Jahren  eingesetzt. Die kerndokumentation zeigt, dass 60% der Patienten MTX erhalten, über die Hälfte davon als Monotherapie. MTX ist der wichtigste Kombinationspartner für andere Medikamente.
Man kann mittlerweile nachweisen, dass höhere Dosierungen möglich und sinnvoll sind. Auch die subkuane Gabe hat sich bewährt wegen besserer Resorption und weniger Therapieabbrüchen.
Patienten mit rheumatoider Arthritis tragen ein höheres Risiko für Infektionen. Nach Studienlage steigt aber das Risiko nicht wesentlich durch MTX, allerdings läßt sich eine Zunahme des Risikos z.B. bei der Gabe von Korikosteroiden, Diabetes melletus, chronischer Lungenerkrankung, Alkoholismus und weiterer nachweisen. Das kardiovaskuläre Risiko von Patienten mit entzündlich-rheumatischen Erkrankungen wird durch MTX vermindert.
Im letzten Jahr konnte ein protektiver Effekt gerade von MTX auf die Entwicklung von Demenz gezeigt werden. Einer meiner Brüder sprach mich vor Jahren einmal zu Demenz bei Rheumatikern an; ich sagte ihm, dass dies eher die Ausnahme sei. Er hatte einen Bericht von einem Pathologen im Fernsehen gesehen, der dies auf die Unterdrückung von Entzündung zurückführte. Ein toller Nebeneffekt, jetzt besonders für MTX nachgewiesen.
Ineffektivität der Therapie mit MTX ist sehr vom Rauchen abhängig; generell sollte vom Rauchen abgeraten werden. Ein geringer Alkoholkonsum ist nach derselben Studie bei MTX nicht nur möglich sondern auch sinnvoll, da die Krankheitsaktivität bei diesen Patienten niedriger war.

Auf Kongressen interessiert vielfach die Innovation, aber der Blick auf bewährte Medikamente bringt ebenfalls kontinuierlich Neues.
